Settle upon the patient's chest in the mounted position, knees drawn tight to the ribs, weight borne low to forestall the bridge and roll.

Slide the right forearm beneath the chin, palm turned downward, so the bone of the wrist rests lightly against the near side of the throat.

With the right hand, seize the left sleeve at the elbow, drawing the fabric across behind the neck to form a closed loop.

Pass the left hand beneath the patient's right arm and grasp the same sleeve near the forearm, securing a second anchor point against the far shoulder.

Rotate the right fist inward so the edge of the forearm bone, not its flat, settles into the carotid hollow just beneath the jaw.

Draw both elbows downward and toward the centerline in a single motion, shortening the sleeve loop and closing the strangle evenly on both sides of the neck.

Hold firm, watching the eyes and the hands. Release the instant the limbs go soft or the tap is given.
